RSTC Medical Statement (dive medical form)

Also: dive medical form, medical questionnaire

The RSTC Medical Statement is a standardized health form used globally. It helps dive instructors decide if you need a doctor’s clearance before your first tank hits the water.

Most shops in Koh Tao or Cebu use this exact document. If you answer ‘yes’ to any medical question, you must see a physician. This process often costs between $50 and $150 depending on the clinic. You cannot skip this step because it protects your life and the instructor’s license.

Do not lie about your history to save time. I once saw a student hide asthma issues in 2018, which led to a scary respiratory incident at 18 m. Always be honest about your blood pressure or heart rate. It is better to spend an extra day at a clinic than to risk a lung injury.
